A "hydrocele testis" is an accumulation of clear fluid in the tunica vaginalis, the most internal of membranes containing a testicle. A primary hydrocele causes a painless enlargement in the scrotum on the affected side and is thought to be due to the defective absorption of fluid secreted between the two layers of the tunica vaginalis . A secondary hydrocele is secondary to either inflammation or a neoplasm in the testis. A hydrocele usually occurs on one side, but can also affect both sides. The accumulation can be a marker of physical trauma, infection, tumor or varicocele's surgery, but the cause is generally unknown. Hydrocele normally is seen in infant boys, as enlarged scrotum. In infant girls it appears as enlarged labia. However, hydrocele is more common in boys than girls. There is a greater chance of infertility if the hydrocele does not show itself until the child has reached adulthood. The younger the patient is when the hydrocele is found and treated, the lesser the chances of infertility. Some beg to differ on infertility due to hydrocele, but the majority agree upon the chances of infertility due to hydrocele is highly likely, unless the patient is an infant or child of pre-teen age. During embryogenesis, the testis descends through the inguinal canal, drawing a diverticulum of peritoneum into the scrotum as it descends. This peritoneal tissue is known as the processus vaginalis. Normally, the communication between the processus vaginalis and the peritoneum is obliterated, and the tunica vaginalis is the tissue that remains overlying the testis and the epididymis. Congenital hydrocele results when the processus vaginalis remains patent, allowing fluid from the peritoneum to accumulate in the scrotum. Bladder exstrophy Bladder exstrophy is a congenital abdominal wall defect that occurs two to three times more commonly in males than females. Widening of the pubic symphysis, or the manta ray sign, is found with bladder exstrophy. Elevated alpha fetoprotein on the maternal quad screen may be the first indication of bladder exstrophy. Nonvisualization of the bladder on ultrasound is an additional sign of bladder exstrophy; however, the diagnosis can be difficult to diagnose sonographically. As low as 25% of patients with bladder exstrophy have a diagnosis made antenatally. Bladder exstrophy can be associated with cryptorchidism, inguinal hernias, and epispadias. OEIS complex, which was not present in this case, includes the congenital anomalies omphalocele, bladder/cloacal exstrophy, imperforate anus, and spinal anomalies. Cloacal exstrophy is a more extensive abdominal wall defect that includes bladder exstrophy, omphalocele, and an abdominal wall defect. Additional congenital abnormalities that result in widening of the pubic symphysis include prune belly syndrome, cleidocranial dysostosis, and osteogenesis imperfecta. A portion of the splenic tissue then descends with the gonad. Splenogonadal fusion has been classified into two types: continuous, where there remains a connection between the main spleen and gonad; and discontinuous, where ectopic splenic tissue is attached to the gonad, but there is no connection to the orthotopic spleen. Patients with continuous splenogonadal fusion frequently have additional congenital abnormalities, most commonly cryptorchidism. The anomaly was first described in 1883 by Bostroem. Since then more than 150 cases of splenogonadal fusion have been documented. The condition is considered benign. A few cases of testicular neoplasm have been reported in association with splenogonadal fusion. The reported cases have occurred in patients with a history of cryptorchidism, which is associated with an elevated risk of neoplasm. Splenogonadal fusion occurs with a male-to-female ratio of 16:1, and is seen nearly exclusively on the left side. The condition remains a diagnostic challenge, but preoperative consideration of the diagnosis may help avoid unnecessary orchiectomy. On scrotal ultrasound, ectopic splenic tissue may appear as an encapsulated homogeneous extratesticular mass, isoechoic with the normal testis. Subtle hypoechoic nodules may be present in the mass. The presence of splenic tissue may be confirmed with a technetium-99m sulfur colloid scan.

Video abstract of case report paper "Mixed gonadal dysgenesis associated with persistent Müllerian duct syndrome – a rare anomaly" published in the open access journal Pathology and Laboratory Medicine International by Baig. Background: Persistent Müllerian duct syndrome (PMDS) is a rare autosomal recessive inherited disorder. It is a type of internal male pseudo-hermaphroditism in which Müllerian duct derivatives persist in the phenotypically and karyotypically normal male. The exact incidence is unknown, but to date, approximately 400 cases have been reported. PMDS is caused either by insufficient amount of Müllerian inhibiting factor or insensitivity of the target organs to Müllerian inhibiting factor. The case presented in this report discusses mixed gonadal dysgenesis, a condition that refers to individuals who usually have a differentiated gonad on one side and a streak gonad or testis on the other side. Case summary: A 27-year-old male patient presented with cryptorchidism and a left inguinal hernia. On exploratory laparotomy, a uterus with bilateral adnexa was noted, and histopathology revealed the features of mixed gonadal dysgenesis associated with PMDS. The closest differential diagnosis for this condition is true hermaphroditism. Conclusion: PMDS is a rare disorder, and is important to diagnose this condition early because 30% of the cases progress to gonadoblastomas like dysgerminoma, yolk sac tumor, and embryonal carcinoma, and hence gonadectomy is necessary. The very rare nature of this condition and grave prognosis merits its reporting.
